Access to health and medical records
Health and medical records held by public health facilities in NSW, such as your NSW hospital records, are maintained by the relevant NSW Local Health Districts. To obtain access to your medical or health records from public health facilities, you will need to contact the relevant local health district.

Government Information Public Access Act 2009 (GIPA)
The Government Information (Public Access) Act 2009 (GIPA Act) fosters public sector transparency and accountability by providing the right to government held information and encouraging government agencies to proactively release their information.

How to access eHealth NSW information
To access information from eHealth NSW, first search our website to see if this information is already available. If it is not, please contact our Right to Information Officer and ask for this information. Your request for information will then be evaluated on whether it:
- is open access information that is readily available. If it is, we will tell you where and how you can get this information
- can be disclosed to you through an informal request, for example, where no third-part personal information is involved
- requires a formal access application, for example, because consultation with a third party is required.
Open access
Under the GIPA Act, eHealth NSW is required to make certain information freely available on its website.

Policy documents
eHealth NSW is subject to and adopts policy directives issued by NSW Health.
Disclosure log
eHealth NSW has not presently determined any prior applications whose details are suitable for publication on a disclosure log. Details of applications suitable for publication will be added to this page as they are received and assessed.
Government Contracts Register
Transparency requirements of the GIPA Act are met by eHealth NSW via the publication of contract details on the buy NSW webpage.
Informal request
If the information you require is not available on our website and doesn’t raise a possible public interest reason as to why it should be kept confidential, then eHealth NSW may be able to release the information to you without the need for a formal access application.
Generally, eHealth NSW will endeavor to release information without the need for a formal access application, unless there are good reasons to require one.

Formal access application
If the information you are seeking is not available on the website and is not otherwise provided by eHealth NSW on request, then you have a right to formally apply for access to specific information.
To make a formal GIPA access application for Government information held by eHealth NSW, the Government Information Access Application Form should be completed and submitted, together with payment of the $30 application fee.
When selecting payment options, please select Ministry of Health as per the instructions of the application form.
Once we have made a decision on your access application, you will be notified of the outcome of your request. If access is not provided, either in whole or in part because there is an overriding public interest against disclosure, you will be advised of the reasons for this decision.
